| Issue: | The manufacturer has seen an increased frequency of dispensing errors on the Phadia platforms due to capsules stuck in carriers during Q3 2021. Small deformations of capsules may occasionally occur during manufacturing. It has been identified that one piece of the production equipment has been damaging the capsules intermittently over a period of time. The deformation is located underneath the flange of the capsule and may cause the capsules to get stuck inside the carriers. |
